Our Services
Visit And Interview AFHs
We regularly visit and interview AFHs, Assisted livings and other long term care facilities to make sure they provide acceptable housing and appropriate care for senior adults. We obtain information such as the level of medication management services provided, sources of payment accepted, primary language spoken by care providers & behavioral problems that can or cannot be met. We also make sure that the long term care facilities are in good licensure with the state of Washington
Assessment
We then do an assessment of the senior family member using a standardized intake form. We discuss recent medical history, type of care the member needs, mental illness such as dementia, activity preferences, & their financial situation
Tour
We accompany the families to tour the different senior living facilities in the preferred locations
After Placement
After placement, we make sure we follow up with the senior member to make sure everything is going on well. We also keep client’s record for a period of 6 years as required by the law
Recommendations
If the senior member decides to stay at home, we recommend in home care agencies and also other companies that assist with things such as cleaning, organizing, and transportation that makes life easier at home
